Your résumé.
Your personal statement.
Your essay.
Professional letter of recommendation, with recommender contact information.
Academic letter of recommendation from your current graduate program, with recommender contact information.
Unofficial graduate school transcript. Please note: An official graduate school transcript will be required if you are offered a position.

Please note the Kaiser Permanente Administrative Fellowship Program is unable to sponsor visas at this time. The organization has the potential of sponsoring work visas for full-time positions post-fellowship.
